Health Forms: Deadlines and annual exclusion notice 

Incoming Freshman Class school physical and immunization records: Due May 1

Notice of Annual Illinois Exclusion Day: October 15,  for all students who are not in compliance with required health forms deadlines for the school year.

Health Form Deadlines: Please send in health forms as early as possible and prior to deadlines. Health Services aren't open during the summer.

Senior Class of 2024 meningitis: immunization record due the first day of school.

Transfer students: school physical, immunization records, and eye exam records: within 30 days of registration.

Dental Form Requirement: Illinois law (Child Health Examination Code, 77 Ill. Adm. Code 665) states that all children entering the ninth grade of any public school shall have a dental examination. The examination must have taken place within 18 months prior to May 15 of the school year. A licensed dentist must complete the examination, sign and date this Proof of School Dental Examination Form. If you are unable to get this required examination for your child, fill out a separate Dental Examination Waiver Form.
