Food Allergies
We request that all students with food allergies have an emergency action plan (EAP) submitted at the beginning of each school year. EAPs provide an individualized plan from a medical provider regarding allergy diagnosis and emergency treatment, including instructions and medication orders. Please have your student's medical care provider complete the emergency action plan below. This form must be signed by a medical provider and a parent/guardian.
Students with known allergies must carry their Epi-Pen with them at all times.
