Media Center Equipment Request Form
This form is for students and staff to request equipment from the HPHS Media Center. Equipment will be assigned based upon availability and scheduled needs. You will receive confirmation of your request and equipment availability will be confirmed via email within 48 weekday hours.

Media Center Support Request (staff only)
This form is to ask for assistance with Media Center related projects, questions, events and lamination. For productions, meetings, and video recordings, please submit the request at least two weeks prior to the event. You will receive confirmation of your request and be informed of the Media Center's availability via email within 48 weekday hours.
