Religious Exemption
If you are requesting a religious exemption from any of the Illinois State required vaccinations, the Illinois Certificate of Religious Exemption form must be completed and signed by a physician. Please submit this form along with the Illinois Child Health Examination Form before October 15th to avoid school exclusion.

Medical Exemption
If your student has a medical contraindication for any of the required Illinois State vaccines, please have your health care provider write a letter stating the following:
- A list of vaccines that are medically contraindicated.
- The reason why they are medically contraindicated.
Should the medical condition of the child later indicate that immunization is no longer contraindicated to the health of the child, the immunization requirements will then have to be met. Please submit this form along with the Illinois Child Health Examination Form before October 15th to avoid school exclusion.
