Senior Meningococcal Vaccine Requirement
The State of Illinois requires that all seniors receive a second dose of the meningococcal conjugate vaccine (MCV4, Menveo, Menactra). If your student received one dose after the age of 16, they do not need a second dose. When your student receives this vaccination, send proof of the vaccination to Health Services.
For your student to be compliant with the senior Illinois state requirement, please submit one of the following:
- Proof of vaccination.
- Proof of appointment for vaccination and proof of vaccination submitted immediately after appointment.
- Illinois Certification of Religious Exemption form. This form must be signed by a physician per Illinois state requirements.
