On August 3 the IC Portal opened for parents/guardians to securely submit vaccination status for their students. Only school nurses have access to student health records in our student information system (Infinite Campus), and they will use this information when situations arise that require knowing who is vaccinated. School nurses will verify the vaccine information provided in the Illinois database for vaccination information I-CARE, the Illinois Comprehensive Automated Immunization Registry Exchange.
District 113 will monitor and publicize vaccination rates for students and staff.
Students of parents or guardians who do not respond will be considered unvaccinated for any situation for which vaccination status makes a difference.
The survey will ask for vaccine type, date and state where administered for dose one, and date and state where administered for dose two. Parents/guardians should not submit the information unless both dose dates can be entered. (Only one date is required for the Johnson & Johnson vaccine). In other words, do not submit the form if your student has not been vaccinated for COVID-19 or has only received one dose of a two-dose regimen. Once the second dose has been received, parents/guardians can go in to submit the survey.
Parents/guardians can securely submit their student’s vaccination status by logging into the Infinite Campus Parent Portal and completing the voluntary vaccination survey in the Message Center. There is a survey in the Message Center for each student in your household. The survey can only be submitted once for each student. Only one parent/guardian needs to submit the survey.
While there is no deadline to complete this survey, it is recommended that parents/guardians submit vaccine information by August 9 to ensure that information is in place to allow Administration to accomplish what the Board determines for mitigation strategies for extracurricular activities and sports.
