We welcome your student to Summer School.
In an effort to properly care for your student during summer school hours, please complete the Summer School Emergency Form and return it to Health Services office.
If it is necessary that your student receive medication during summer school hours, prescription and/or over-the-counter non-prescription, District 113 requires the District 113 Permission to Administer Medication form be completed. Any medication to be administered must be supplied directly from the parent/guardian to the Summer School Nurse. Please read the Procedures for Administering Medication prior to submitting the permission form (English, Spanish).
Students may not carry medication during summer school, except:
- Asthma inhalers
- Epinephrine auto-injector
- Diabetic medication/supplies
If your student has a chronic health condition, please complete and submit the appropriate Action Plan. Action plan templates can be found here and must be signed by a licensed medical provider and parent/guardian.
