April 21- Wednesday, May 7, 11:59 PM Advance registration
is required for all participants.

Remember to print your registration receipt.

Students requesting financial assistance, please choose scholarship when registering.

Student ID and a printed registration form are required for payment and/or ticket pick-up at the Bookstore no later than Thursday, May 8, 3:00 PM

INSTRUCTIONS: (Please follow these instructions to register and choose a payment type)

1. Go to our Online Payment Store: http://dist113.revtrak.net
►Click the Highland Park High School link on the left side of the screen.
►Click the HPHS Activities link on the right side of the screen.
►Click the Prom link.

2. Enter your last name and student identification number

3. Enter your grade level

4. If applicable

a. Enter your guest grade level

b. Enter your guest school

c. Complete guest info boxes

5. Enter Emergency Parent Contact Info for you and guest (if any)

6. Click BUY NOW – please allow for processing time

7. View Cart by scrolling over and check totals on the right hand side – if you are paying at the Bookstore your total will list FREE

8. Click GO TO CHECKOUT – or read on-screen instructions for any needed updates

9. Sign in as a New Customer or as a Returning Customer with your password

10. Enter Billing Information – this applies even if you are paying at the Bookstore with cash/check

11. Click VERIFY MY INFO

12. Verify billing info - purchaser, address and credit card (if using) and click COMPLETE ORDER

13. Print your receipt to take to the Bookstore to pay and/or pick up your ticket(s)
Deadline for ticket pickup is Thursday, May 8^th at 3:00 PM.

Guest Information:
If you are bringing a guest from a school other than HPHS, you must know his/her full name, ID# and school name.

A guest information form must be completed and submitted when you pick-up your ticket. The form is available on-line. You should print the page to be completed by you, your parents, and your guest’s school before picking up your tickets.

If your guest is no longer in school, write “none” for school name. Your Dean will then need to sign the form.

Scholarship/Financial Assistance Information:
On-line registration still required.
Students who are unable to purchase a ticket based upon financial need may apply for complimentary ticket by completing the financial assistance form and turning the sheet in to the Activities Office. You will be notified about your scholarship application via school e-mail. Students who are requesting financial assistance and bringing a guest are still required to pay for the guest ticket and should present the registration information and payment at the bookstore for the ticket purchase and ticket-pick-up.

Table Seating:
The table reservation information and form will be available on line after April 21^st. Table seating by group will take place periods 4-7 only on Monday, May 12^th in the HPHS library. Each student in the group must have purchased a ticket by May 7^th. Groups may send ONE representative to the library to reserve a table
