Welcome
to the Deerfield High School Community
If
you have not already done so, please make an appointment to meet with a counselor
for registration. The phone number to set up an appointment is (847) 405-8460.
At the appointment time, please be prepared to share information regarding
your academic experience and discuss your preference for courses.
Both student and parent must be present at this meeting.
If you are in Deerfield and would like to review a curriculum guide
prior to your appointment, please come to the high school and ask for a copy
or you

information.
When
you come to register, please bring:
- Proof
of residency. The following
documents are required: copy of lease, current tax bill, or closing statements,
AND a copy of a current gas or electric bill.
- Birth
Certificate as required by law.
- The
following forms, completed: the Registrar Letter (Records Release), the
Student Information Form, and the Home Language Survey.

listing of courses and grades from your previous high school.
- Achievement
test scores (if you have access to them).
- Health
Service Forms.
- (Illinois
Residents Only) Student
Transfer Form (ISBE 33-78) if student is enrolling from another public
secondary school in Illinois.
This form verifies if a student changing from one public high school
to another public high school within Illinois is in good standing at his/her
high school and whether medical records are current. This form must be presented at the time of registration.
Eligibility
for registration must be approved by an Assistant Principal before the registration
interview may begin.
The
registration interview will last approximately 1½ hours.
In addition to the registration interview, you may be asked to return
to Deerfield High School for placement testing.
